PARTNERS IN EXCELLENCE CHILDREN’S CASE MANAGEMENT IS HERE TO HELP!

 

WHAT IS CHILDREN’S CASE MANAGEMENT?

Parents know their children best.  Case Managers know the community providers and services.  The Mental Health Targeted Case Management team partners with parents to support and improve the day-to-day mental health and development of each child.  By working together, we hope to be a life-changing experience for both the child and the family unit.

 

CASE MANAGER’S ROLE – HOW WE HELP

Your case manager will provide support for you and your child to ensure your child’s needs are being met.  You will meet with your case manager in person monthly with regular communication to keep everyone updated.

What your case manager does:

– connect your child with services and community supports

– navigate funding sources to support the needs of your child

– advocate for your child and attend meetings with other members of their care team

– coordinate with all care team members and service providers so everyone is on the same page

– find answers to your questions and concerns

 

IS MY CHILD ELIGIBLE?

Individuals, ages 0-21, living in Winona County with a qualifying mental health diagnosis are eligible.

Partners Case Management must comply with the criteria given by the Minnesota Department of Human Services website regarding eligibility.  DHS Case Management

 

WHAT IF MY CHILD DOESN’T HAVE A DIAGNOSIS?

Partners Case Management will assist you and your child in receiving the necessary assessments by a mental health professional.

 

I WORK DURING THE DAY AND CAN’T MEET WITH A CASE MANAGER

Your case manager will accommodate your schedule and can meet with you and your child in the morning before work/school or in the evenings.

 

I DON’T HAVE TRANSPORTATION TO MEET WITH A CASE MANAGER

Partners Case Managment will go to you.  There are no requirements to meet in the office at any time.  Your case manager will always be available to meet at your home or out in the community.

 

WHO CAN MAKE A REFERRAL?

Referals can come from anyone in your child’s life.

– Parents/Caregivers/Guardians/Family Members

– Teachers/School Administrators

– Mental Health Provider

– Social Workers

– Pediatrician/Family Physician

– Religious Leader
