Partners In Excellence Case Management team partners with families at our Winona location to connect children with mental health diagnoses and their families, with the providers and services they need.
This service is only available at the Partners Winona location.
Parents know their children best. Case Managers know the community providers and services. The Mental Health Targeted Case Management team partners with parents to support and improve the day-to-day mental health and development of each child. By working together, we hope to be a life-changing experience for both the child and the family unit.
